Responsibility for service The computer equipment described in Paragraph 1 requires service to effect a repair, or to modify its hardware configuration. Recommendations for service made by Network and Academic Computing Services, Distributing Computing Support (NACS-DCS) staff may have been provided to me as guidance for what repairs or modifications should be made. Full responsibility for what repairs or modifications are to be made rests solely with me. 3. Request for service I hereby request that NACS-DCS staff act on my behalf to perform the services referred to in Paragraph 2. 4. Billing rates for service Hardware services performed on computer equipment are excluded from the standard NACS-DCS system administration contract. All hardware-related services are billed in full increments of 1/2 hour at the standard NACS-DCS time and materials rate. Recharge number:_______________________________________________ 5.
